Just passed an MOT (thank the E30 gods). The tester said the C02 output is suspiciously low (0.146) and I have noticed that at idle, the engine does not run smoothly/stutters a bit.

Could this point to me running a bit lean?

You probably mean CO?? CO and CO2 are two completely different gasses.
First port of call will be a set of reconditioned injectors.

Yep, I did mean Carbon Monoxide!

Would it be best to find replacement injectors or just get my stock ones cleaned/flow tested?

Go to our traders' forum, and look for "injectortune".
If you can live without the car for the time it takes to get them rebuilt and returned, then send of your existing ones.
If you want to cut the 'down time', then obtain a second hand set, and send those off.
