just need to know what causes the fan to work on speed 4 only
and where is it in the car ?

I had the same on my touring, and the heat resistor was fine, but the little overheat switch on it had tripped.
I welded it and now all works fine !!!
You have to remove the cover behind the c101 plug which is running below the windshield in the engine compartment.
The heater resistor board is just clipped in, grab it and pull it out. You can resistors with a basic Ohmmetre, if infinity, it's screwed, if you have resistance value, it might be ok, so just check the overheat switch which is across the resistor board.

Nope, since 1-2-3 is controlled by the resistor, speed 4 is direct no resistor.
But i think your resistor could be damaged since speed 1-2 is the first to go.
Im guessing the cable that goes to speed 4 is damaged, so you would have 2 things to fix.

I had the same problem - working only on speed 4. Mine was cured by taking out & cleaning the little coil that resides on the fan behind fan cover, behind the firewall (accessed from the engine bay).

Most of these sort of problems are caused by a fan motor that is beginning to seize. The motor draws excess current and starts to cook various things in its supply path.
Take the motor out (can't be done properly in situ) and lubricate it properly with light oil. Power it up out of the car, and get it running absolutely freely before putting it back in.
Then you can sort the other damage.
