ICV-is it dirty or not got an airtight connection?
AFM-no unmetered air getting in through a split in the pipes/TB tightened correctly?
ECU-did you give it the minimum 10 mins sitting at idle to get base settings?
Fuel pressure too high?
sorry thats all I can think of-hope you get it sorted!

air flow or even ecu. Is the loom damaged?

No one's mentioned the obvious - throttle cable too tight.
Chwck there is a little slack on idle.

what does it do when you try and rev it up?? does it go smothley or does it only go so far, coz i remember when i done 1 a while ago the syptoms sound just like yours, it was a verry fcuked up idle where it was just going up and down, and when i tryed to rev it up it seemed like the limiter had been brought down to about 4000rpm, and i tracked it down to the conections next to the reset thing on the engine, i think one is the timing sensor, and the other is erm.....

all i had done was connected them up the wrong way round

the other is the oil pressure switch
i thought somthing similar but those two plugs are the opposite way up to each other and with a straight engine swap would probably not be touched. Although it would explain the increase in rev's

does the throttle close all the way ? not just the spring on top.
if so what happens when you pull the icv out and block the holes ? (hold the throttle open slightly to compensate but leave the loom connected)

High idle and over fueling sounds like blue temp sensor. You sure you got the right connector on it?
