Omg!
I spoke to the host this morning who were looking into it (last spoke mid-last week) and they came to the conlusion it was our code (which hasn't been altered for months) yet it does appear to be working, I don't use email notification but one of my new scripts has started to work..

Topic notification replies. When somebody posts a reply to a topic you have posted on, you get sent an email with a link. Its an option in your account i think.
