More woes :( ....

1991 316i M40 engine (137k)
Drove to London today (40 miles) - didnt miss a beat. Drove back this evening at same speed - sitting at 72 on motorway and it began to hesitate, in a way that I can only describe like it was not getting constant fuel supply or maybe a missfire.
The nose went down slightly when it was doing this so it obviously lost acceleration, but kept picking up and repeating on and off for about 10 miles.
Once off the motorway it was fine - revved up to 4k without any problem.
Only other trouble Im having is taht it judders at 2k rpm (Throttle position sensor issues I presume).
Since getting it I have:
Replaced: ICV, Cambelt, Dizzy cap, Spark plugs, Inlet manifold gaskets, AFM, blue temp sensor.
Any ideas gratefully received.
