Play in the rear wheel- Diagnosis please

If i jack the rear up, and leave the car in gear, you can rotate the wheels left to right/right to left, about 25mm. What is the cause, and what to do to rectify this.

If your doing it with the handbrake off but in gear then a certain amount of play is normal in the diff/gearbox.
If the handbrake is on but can still rotate 25mm then id say the handbrake shoes are out of adjustment/worn or snapped/fallen out securing pins.
